01Why the list is this short

We would rather publish a list of one than pad it.

A page like this could easily run to a dozen entries. It would need to include supplements, and we are not willing to rank those beside an authorised product as though a reader were choosing between comparable things. The short version: the products in that category that demonstrably work tend to work because they contain undeclared prescription drugs, which is both the reason they are effective and the reason they are dangerous — you cannot dose, and you cannot avoid, an ingredient nobody has told you about.

The gel evaporates, cooling the glans and then rewarming it, and that thermal change stimulates nerve endings. Physics rather than pharmacology.

03Who it is really for

If you take nitrates, this matters more than it looks.

A man taking nitrates for angina cannot take sildenafil, tadalafil, vardenafil, avanafil, or any compounded combination of them. The interaction can drop blood pressure catastrophically and the contraindication is absolute — it is the single most important safety line on this site.

Historically his options were a vacuum device or injection therapy. A product containing no drug does not create that interaction, and that is a real addition rather than a marketing one.

Hold the effect size loosely, though. The main comparator study was unblinded and enrolled 96 men, and unblinding matters more than usual when the mechanism is a sensation the user can feel.

04The uncomfortable part

Wanting to skip the conversation is the most understandable thing on this page.

A prescription means a conversation, and a conversation means saying it out loud. We are not going to lecture anyone for searching this. But an over-the-counter product examines nothing, and this symptom is frequently the first sign of vascular disease — it can precede a cardiac event by years.
