Our assessment
Take the disclosure at face value first, because Ro makes it themselves: this is a compounded product and it is not FDA-approved. Compounding is legal and legitimate — it exists so pharmacists can prepare a formulation an individual patient needs. It does not carry the manufacturing and content verification that sits behind an approved drug.
The clinically interesting part is the combination. Sildenafil and tadalafil are both PDE5 inhibitors — the same drug class, acting on the same enzyme, by the same mechanism. Putting them together is not a standard of care, is not in the American Urological Association guideline, and has not been established as a combination in randomized trials. The individual molecules are extremely well studied; this pairing of them is not.
Because both agents lower blood pressure through the same pathway, combining them is a pharmacological reason for caution rather than a reassurance. That matters most for men taking nitrates, where any PDE5 inhibitor is absolutely contraindicated, and for men on alpha-blockers, where dosing needs care.
